Job Summary:

The Hospital In-Reach Occupational Therapist (Home First Approach) is responsible for delivering patient-centered occupational therapy services with a focus on enabling patients to return home as soon as possible. This role emphasizes early intervention, rapid assessment, and coordinated discharge planning to support patients’ safe and effective transition from hospital to home.

Key Responsibilities:

1. Patient Assessment and Evaluation:

  •  Conduct rapid and comprehensive assessments of patients’ physical, psychological, and environmental needs with an emphasis on their ability to return home.
  • Develop individualized treatment plans aimed at optimizing patients’ independence and readiness for discharge.

2. Home First Approach Implementation:

  • Advocate for the Home First philosophy, ensuring that discharge planning prioritizes patients returning to their own homes.
  • Collaborate closely with patients, families, and the multidisciplinary team to create and implement discharge plans that support home-based recovery.
  • Assess home environments (through home visits or virtual assessments) to recommend necessary modifications or equipment.

3. Therapeutic Intervention: o Implement evidence-based occupational therapy interventions that focus on activities of daily living (ADLs).

  • Provide training and education to patients and caregivers on adaptive techniques and the use of assistive devices.

4. Discharge Planning and Coordination: o Actively participate in multidisciplinary discharge planning meetings.

  • Coordinate with community services, social workers, and other healthcare providers to ensure seamless transitions from hospital to home.
  • Provide detailed recommendations and plans for post-discharge therapy and support services.

5. Documentation and Reporting: o Maintain accurate and timely documentation of assessments, treatment plans, progress notes, and discharge summaries.

  • Ensure compliance with hospital policies, regulatory requirements, and professional standards.

6. Professional Development and Education:

  • Stay current with best practices and advancements in occupational therapy and the Home First approach.
  • Participate in continuous professional development activities.
  • Educate hospital staff, patients, and families on the Home First approach and its benefits.

7. Quality Improvement:

  • Participate in quality improvement initiatives related to discharge planning and home-based care.
  • Collect and analyze data to evaluate the effectiveness of the Home First approach.
  • Contribute to the development and implementation of policies and procedures that support early discharge and home-based recovery.

Qualifications:

• Education: Bachelor’s or Master’s degree in Occupational Therapy from an accredited program.

• Licensure/Certification: HCPC

• Experience: At least 2 years of clinical experience, preferably in a hospital or acute care setting. Experience with discharge planning and home-based care is highly desirable.

• Skills:

  • Strong clinical assessment and intervention skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Proficiency in electronic health records and documentation.
  • Strong organizational and time management skills.
  • Knowledge of community resources and home-based care options.
  • Carry out moving and handling assessments.
  • Capacity to work in a fast-paced hospital environment and handle stressful situations.

Working Conditions:

  • Primarily hospital-based with regular home visits.
